How UTMB reduced readmission rates using a data-driven, multidisciplinary approach

Efforts undertaken by UTMB to reduce 30-day readmission rates are the focus of this report. Launched in 2016, the initiative significantly improved the care transition process and also enhanced both the patient experience and outcomes. By 2017, UTMB was able to reduce 30-day all-cause readmissions by 14.5 percent.
