"Before you go, make a list of all the medications you are taking. Include the name and dosage of the drug, the name of the physician who prescribed the medication and why you are taking it. Also include the name and telephone number of your pharmacy so your doctor can call in prescriptions if necessary. Next, list your allergies - the substance you're allergic to and your reaction.
